Write a complete description of the proposed change or alteration and include a drawing, photograph, catalog picture, or specification where appropriate.

Your application will be reviewd by the appropriate committee and a recommendation presented to the Board for their approval, disapproval or recommendation. Normally, the Application process may take up to 45 days. Applications which require additional review or study may take longer. Please allow sufficient time to obtain city permits and application approval before beginning your change.

I UNDERSTAND the rules concerning the proposed improvement and that the final decision regarding my request will be made by the Board of Trustees. I further understand that should the Association need to remove any part of this improvement in order to maintain or repair any aspect of the Association property, it will be my sole responsibility to repair and/or replace the improvement.
I UNDERSTAND that if a Building Permit is needed for this improvement, it is my responsibility to acquire such a permit. The project cannot commence until this permit is issued. I will supply a copy of the permit to the Board. Copies of the Builder's Worker's Compensation, P.D. and P.L. insurance certificate(s) will also be supplied. Furthermore, I will notify the Board of Trustees for inspection and fianl approval within fourteen days of completion of the improvement.
I ACKNOWLEDGE that the installation of this improvement, and, unless this is a landscaping improvement, the perminant maintenance thereof is my sole responsibility. I understand that once completed, the improvement will require the final approval of the Board of Trustees. I further understand that any collateral damage caused by this installation is also my sole responsibility to repair and replace as determined by the Boad of Trustees.
I FURTHER ACKNOWLEDGE that should any transfer of title to my property occur, it will be my sole responsibility to notify my successor and/or assigns in title of their responsibility to maintain the aforementioned installation.
